Proof of entity filing with state: articles of organization and operating agreement with federal and state business I.D. numbers

 

List of owners, social security numbers, initial investment, and ownership

percentages

 

Copies of previous 2 to 3 years tax returns

 

Back up copy of accounting software file with reconcililations. If you do not use an computerized accounting program then provide a listing of:

 

                   *Copies of and credit card statements with business                                            transactions 

                   *1099's and W-3's issued by your business 

                   *Business expenses paid out of personal money or credit cards 

                   *List of assets (such as vehicles, equipment, etc) with receipts                         *Year end statement(s) on all business loan(s) showing ending                                    balance(s) and interest paid during the year

 

 

 

for nonbusinesses (single, sole proprietors, married filing joint; or separate, widower, and self employed persons):

  • Dependants’ Social Security numbers and dates of birth

  • Canceled Debt Amount/1099-C

  • Statements on the sales of stocks or bonds/1099-B

  • Interest and dividend income/1099-INT/1099-DIV

  • Lottery or gambling winnings/losses

  • State refund amount/1099-G

  • Income and expenses from rentals

  • Alimony paid or received

  • Record of purchase or sale of residence

  • Medical and dental expenses

  • Real estate and personal property taxes

  • State or local taxes paid Estimated taxes or foreign taxes paid

  • Cash and non-cash charitable donations
